AIRWAY & EQUIPMENT SKILLS
Technique films from the i-gel manufacturer and medical-education channels, each with what the FIFA Emergency Care Manual says. Where a film differs, follow FIFA and the device instructions.
FIFA ECM KEY POINTS
- Trauma: jaw thrust is preferred — it avoids moving the cervical spine. Head-tilt/chin-lift is contraindicated with suspected trauma.
- Middle and ring fingers under the angle of the mandible, lift upwards. Only the jaw moves — not the head, midface or neck.
- If the person holding MILS cannot jaw-thrust without moving the neck, stop: a second person protects the spine.
- Never put fingers in a player's mouth — you cannot “swallow your tongue”.
FIFA ECM KEY POINTS
- Size from the midline of the incisors to the angle of the mandible.
- Usually size 3 (orange) adult man, size 2 (green) adult woman; size 4 (red) or 5 (purple) for tall players.
- Insert upside-down and rotate 180°; flange rests at the lips. Flange pushed out = too big.
- Do not use if the player coughs or gags, with a choking foreign body, or with clenched teeth (seizure).
FIFA ECM KEY POINTS
- Best-tolerated adjunct — can be used with a gag reflex.
- Choose one slightly smaller than the nostril: usually size 7–8 man, 6–7 woman. Safety pin if the type needs one.
- Lubricate; pass straight BACKWARDS along the floor of the nose, never upwards. Gentle twist; never force — try the other nostril.
- Relative contraindication: suspected base-of-skull fracture. Caution with nosebleed.
FIFA ECM KEY POINTS
- Preferred adjunct in apnoea and cardiac arrest — allows continuous compressions.
- Size by weight: size 4 (green) 50–90 kg, size 5 (orange) over 90 kg.
- Lubricate, cuff outlet towards the chin, glide along the hard palate until definite resistance; teeth on the bite block. Tape maxilla to maxilla.
- Not a definitive airway. Not tolerated if the player is coughing / has a gag reflex.
FIFA ECM KEY POINTS
- Attach the bag with reservoir to oxygen at 15 L/min.
- Face mask + OPA/NPA is a two-person technique and CPR pauses for 30:2.
- With an i-gel: continuous compressions, one breath every 10th compression (~10/min). Leak → pause for 30:2.
- Squeeze, then let the bag refill. Look for symmetrical rise and fall of the chest.
FIFA ECM KEY POINTS
- One person seals the mask (and holds the jaw), the other squeezes the bag.
- This is why FIFA prefers the i-gel in cardiac arrest: one person can hold it and ventilate.
FIFA ECM KEY POINTS
- Suction and oxygen at 15 L/min must both be available pitchside.
- Use for blood, secretions or vomit as part of airway management — before moving on to breathing.
- Apply oxygen as soon as it arrives; do not wait for the primary survey to finish.
FIFA ECM KEY POINTS
- Neutral head: line from chin back to sternomastoid, measure fingerbreadths down to trapezius.
- Between two sizes → start with the SMALLER one.
- Airway obstruction is a contraindication — keep MILS instead. Recheck the player after fitting.
- A collar alone does not immobilise: keep MILS until blocks, tape and a device are on. Treat the collar as part of extrication.
FIFA ECM KEY POINTS
- Four people + a fifth to place the device. The person at the HEAD controls the move and the count.
- Head: MILS. Chest: opposite shoulder + hip. Pelvis: waist + under the knee. Legs: lower leg + ankle.
- Say how far: 90° full log roll vs 15–20° tilt for a scoop.
FIFA ECM KEY POINTS
- FIFA prefers a scoop for trauma: only ~15° of movement vs 90° log roll for a board.
- Basket in line with the feet 2–3 lengths away; scoop laid out, extended and split between basket and player.
- Trauma: scoop stretcher and basket. Cardiac arrest: a long spinal board (a scoop can also be used), with the AED strapped on as one unit.
